your legal name
your date of birth
your year of graduation and/or years you attended Pendleton
a telephone number and/or e-mail address where we can contact you if we have any questions
your current postal address
the address and/or fax number of the college(s)/coach(es), to which the transcript should be sent.
Please note the following: If your transcript is sent to a coach it will not reach the undergraduate admissions office.
Also, an admission office with a P.O. Box address cannot be sent via
UPS or FedEx.
